Mental Health Care

Inpatient and Outpatient Services

As people age, their health needs often become more complicated. Medical problems, such as arthritis and high blood pressure, are common, but so are emotional challenges, including the loss of a spouse and/or living on a declining income. These dilemmas can combine to rob seniors of their health and independence, but help is available.

Inpatient Services


Specialists at the Geropsychiatric Inpatient Medical/Psychiatric unit of Levindale Hebrew Geriatric Center and Hospital treat not only the physical challenges that patients face daily but their emotional challenges as well. Mental health issues can include depression, dementia, agitation and behavioral issues.

Our goal is to help patients reach their maximum functional level, and to live as independently as possible after discharge. Geropsychiatric rehabilitation combines the existing services of internal medicine, rehabilitation, and psychiatry in an integrated, focused program.

Outpatient Services


Levindale's Geropsychiatric Program doesn't stop there. We have two short-term outpatient programs that help the elderly in the community. Levindale's Partial Hospitalization Program is open to those who want to avoid inpatient hospitalization and to those just out of the hospital who still need close monitoring and supervision. Our Outpatient Program follows up with a less rigorous curriculum.

Partial Hospitalization Program


The Levindale Gero-Psychiatric Partial Hospitalization Program (PHP) is a short-term outpatient program designed specifically for older adults who are experiencing behavioral or emotional difficulties, but do not require inpatient treatment. In addition, PHP offers ideal transitional treatment as needed upon discharge from inpatient psychiatric care. This extended support greatly reduces the possibilities of relapse.

Intensive Outpatient Program


The Levindale Gero-Psychiatric Intensive Outpatient Program (IOP) is a short-term outpatient program designed specifically for older adults who are experiencing behavioral or emotional difficulties, but who do not require (or no longer require) the intense level of psychiatric care provided by inpatient or partial hospitalization programs. IOP is ideal for stabilizing those individuals with a history of frequent relapse.

Care Provided at Outpatient Programs


Both Levindale's  PHP and IOP programs have intensive treatment in a therapeutic community of peers. The programs utilize individual, family, and group therapies, as well as medication management, to help each client work toward his or her specific goals.

Each client receives an individualized treatment plan that reflects his or her goals and objectives. Referring professionals are encouraged to participate in treatment planning.
Discharge planning, initiated upon admission, ensures optimal patient support and follow-up after PHP treatment goals are met.

PHP focuses on the techniques necessary to cope with daily life and helps in readjusting to home and community. The IOP program targets stabilization, promotes achievement of realistic goals and increases compliance with treatment a plan.

Levindale's outpatient mental health services have a multi-disciplinary team of caring and experienced behavioral health specialists that include:

Medical director who is board certified

  • Consulting psychologist
  • Clinical social workers
  • Clinical nurse specialist
  • Certified psychiatric nurses
  • Consulting occupational therapist
  • Expressive arts therapist

Our programs run Monday through Friday. Daily attendance for participants varies based on individuals needs. However, some may attend fewer days depending on their treatment goals. Lunch and snacks are provided. Every effort is made to provide transportation.

Outpatient Mental Health Clinic


Nurses and social workers operate Levindale's Outpatient Mental Health Clinic for seniors unable to access clinical services. These outreach services are ideal for bedridden people who have mood disorders, are recovering at home from surgery and illnesses, or have fears related to leaving the house.

Senior Needs Assessment Program (SNAP)


If you are not sure if you or your loved one may need help to cope with the mental health challenges that are being faced, Levindale's Senior Needs Assessment Program (SNAP) may be just what you need.

SNAP provides a comprehensive assessment that addresses the medical, psychological and social issues that may be interfering with the individual's health and ability to function. When each assessment is completed, recommendations and referrals to the appropriate specialized services are made.

The SNAP process begins with a phone screening, which is followed by a home visit or an on-site visit at Levindale. A clinical team meets to discuss each participant's needs to maximize their independence and well being. The assessment includes the areas of:

  • Physical and mental health
  • Behavior concerns
  • Safety issues
  • Social well being

Once an evaluation is completed, recommendations and referrals are made to appropriate and specific services that meet each person's needs. They may include:

  • Rehabilitation
  • Neurology
  • Urology
  • Ophthalmology
  • Gynecology
  • Dermatology
  • Nephrology
  • Gastrointestinal
  • Internist
  • Dental
  • Senior housing
  • Pharmacy 

Referrals


Outside referrals are welcome from physicians, behavioral health professionals, assisted living/group home facilities, retirement communities, nursing homes, family members, and other involved people.

Project LIGHT


Levindale also offers free depression screenings and follow-up monitoring in the community through Project LIGHT .

Levindale's Mental Health Programs


As you can see, a continuum of care is provided with many agencies that offer community resources, including Sinai Hospital and Jewish Family Services.
